ABOUT LEX CHIP: A Community Health Improvement Plan (CHIP) is a long-term, collaborative effort to address the health needs of a specific community. It’s developed based on the findings of a Community Health Assessment (CHA), which systematically gathers and analyzes data about the health status of the community. The CHIP outlines strategies, goals, and action steps to improve the health and well-being of residents. This may involve addressing issues like access to healthcare, chronic disease prevention, mental health, substance abuse, and environmental health concerns. Essentially, a CHIP serves as a roadmap for improving community health, guiding resource allocation, and fostering collaboration among various stakeholders, including healthcare providers, community organizations, government agencies, and residents themselves.
